Record Identifying Information for a Reunification Assessment

The reunification assessment is a structured process to review and document the child’s readiness to leave an out-of-home placement. The assessment includes a review and documentation of past and present  child safety, compliance with court orders, family conditions and dynamics, resources, strengths and protective capacities, child vulnerability, and interventions needed. Process Record a Reunification Assessment

Record identifying information to a reunification assessment as follows:

  1. From the Home screen, click the Case tab.

  2. Click the Workload tab. The Case Workload screen appears.

  3. Click the case ID Number link of the case you want to update. The Case Overview screen appears.

  4. Click the Reunification Assessment link in the navigation bar on the left. The Reunification Assessment(s) screen appears. It lists any assessment records that have been recorded for the case.

  5. Click the Add Reunification Assessment button. The system defaults to the Identifying Information tab screen.  

  1. In the Children Considered for Reunification section, click on the Add Child Consideration button to identify the child(ren) to be reunified. At least one child must be added in order to save the assessment record.

  2. Click on the Add Adult Consideration button to identify the adult(s) to be reunified.  

  1. Click Apply to stay in the record, and continue recording details of the reunification assessment. Click Next Task below to continue the process of recording details to the Reunification Assessment record.  

If you click Save you will be returned to the Reunification Assessment(s) screen. The record will display the status of In Progress. You will click the Edit link next to the Reunification Assessment to re-enter the record in progress.
