The reunification assessment is a structured review to document the child’s readiness to leave an out-of-home placement. The assessment includes a review of child safety, compliance with court orders, family conditions and dynamics, resources, strengths and protective capacities, child vulnerability, and interventions needed.
View a reunification assessment as follows:
From the Home screen, click the Case tab.
Click the Workload tab. The Case Workload screen appears.
Click the case ID Number link of the case you want to update. The Case Overview screen appears.
Click the Reunification Assessment link in the menu on the left. The Reunification Assessment(s) screen appears. It lists the assessment records that have been recorded for each child in the case.
Click the View link next to the name of the child(ren) whose reunification assessment you want to view. The system defaults to the Identifying Information tab.
Click each tab to view the details. There are four tabs:
Identifying Information displays the child(ren) and adult(s) to be reunified.
Past & Present Safety displays a brief assessment of how well the family has addressed safety concerns.
Readiness displays an assessment of how prepared the child(ren) and adult(s) are for reunification.
Decision displays the reunification decision and explanation.
Click Cancel to return to the Reunification Assessment(s) screen.
