Record Case Plan Participants

Process: Record a case plan

The Identifying Information screen allows you to identify the child and adult case members who will be receiving services and working toward achieving the goals identified in this case plan. The system automatically fills in the participants from the most recent, approved family assessment associated with this case. If there is no family assessment, or if you want to update participant information, you can record participant details on the case plan.

Record case plan participants as follows:

  1. From the Home screen, click the Case tab.

  2. Click the Workload tab. The Case Workload screen appears.

  3. Click the ID Number link of the case you want to update. The Case Overview screen appears.

  4. Click the Case Plan link in the menu on the left. The Case Plan Filter Criteria screen appears.

  5. Click Add Case Plan. The Identifying Information screen appears.

  6. Select Case Plan Type Code from the drop down.

  7. Click Add Child button. The Available Case Participants screen appears. It displays the children who are case members. Select the check box next to each child who is a participant in this case plan. You must select at least one child as a participant in this case plan.

  8. Click OK. You are returned to the Identifying Information screen.

  9. Select Child's Permanency Goals from the drop down next to each. Enter Court Case/ID # .  

  10. Click Add Adult. The Available Case Participants screen appears. This time displaying are the adults who are case members. Select the box next to the adults that are participants in this case.

  11. Click OK. You are returned to the Identifying Information screen.

  12. Complete the fields in the Additional Information field that apply to this case plan.

  13. Click Next Task below to continue.
