Enter the number of hours the worker receives. This field is required.

Course Instructor Name

Enter the first and last names of the person who taught the course. You can enter up to 30 characters. This field is required.

Hours Type

Select the type of credit hours the worker receives upon completion of this course. This field is required.

Enter any additional information about this course. You can enter up to 2,000 characters.
