Perpetrator search criteria screen

This screen allows you to search the system to see whether an applicant associated with a child care provider licensed by the State or agency has a history as an alleged perpetrator in CA/N reports that were determined "Substantiated" or "Indicated."

You must enter all of the search criteria fields to ensure an accurate match. The system returns exact and probable matches.

The Not Found Letter link allows you to print the letter informing the person of the search results.

How do I...

Fields on this screen

Last Name	Enter the person's last name. You can enter up to 35 characters. This field is required.
Sounds Like	Select the check box if you want the system to include last names that sound like the one you entered.
First Name	Enter the person's first name. You can enter up to 35 characters. This field is required.
SSN	Enter the person's social security number. This field is required.
DOB	Enter the person's date of birth. This field is required.
Gender	Select the person's gender. This field is required.
