Motion details, (create motion) screen

This screen allows you to view, add, or update the details of a motion in Ohio SACWIS. Complete this screen to create a new motion record and generate a document that you will file with the court. (See About court information.)

If you created a motion record in error, you must end the motion record and select Created in Error in the Reason for Ending Motion field. However, you cannot mark any legal action as created in error when another legal action is currently linked to it.

Motion Filed By	Click Person Search, then select the name of the person who is filing the motion with the court.
Motion Created By	Click Person Search, then select the name of the person who is initiating the complaint.
Court Case Number	Enter the court case number. The court case number is a court-assigned number that includes all children in a family.
Reason for Ending Motion	If an existing motion is not to be filed with the court, select the reason for ending the motion.
Type of Motion	Select the type of motion. This field is required.
Sub-Type	Select the secondary type of motion. The items that appear in this drop-down list are a subset of the selection in the Type of Motion field.
Date Submitted	Enter the date the motion is being submitted to the court. Use the format MM/DD/YYYY
File Stamp Date	Enter the court's file stamp date. Use the format MM/DD/YYYY. Once you enter and save this date, no further changes may be made to this motion record. You can, however, amend or end the motion.
Method of Motion	Select the method of motion. An oral motion is made during a court proceeding. A written motion is documented and filed with the court. This field is required.
Motion Supported by Affidavit	Select either Yes or No to indicate whether this motion is supported by an affidavit.
Motion To	Enter the statement describing the intent of this motion. You can enter up to 3,000 characters.
Motion States	Enter the text of the motion. You can enter up to 3,000 characters.
Best Interest Motion Statement	Enter an explanation of why the requested disposition is in the best interest of the child(ren). You can enter up to 3,000 characters.
Reasonable Effort Determination Motion	As applicable, enter a description of the reasonable efforts the agency made to prevent placement; the condition upon which any decision for removals was mad; attempts to provide the parent, guardian, or custodian with notification of the child's removal; or attempts to provide the parent, guardian, or custodian with notification of any court hearings. You can enter up to 3,000 characters.
