This screen allows you to view, add, or update the details around a
participant's case plan signature and agreement or disagreement with the
case plan.
Select
the case plan participant for whom you are recording signature
details. This field is required.
Non-Case
Member
Click Person
Search, then locate and select the non-case member
whose signature information you want to record.
The case plan must be signed by the attorneys, GALs, CASAs, and
so on, as well as by any other non-case member who has been
added as a party to the case plan but who is not a case member
would need to be added.

Select Yes or No
to indicate whether you have obtained a signature from this
case participant or non-case member.

Date
If you selected
Yes in the Signature Captured
field, enter the date on which you obtained the signature.
Use the format MM/DD/YYYY.
Reason
If you selected
No in the Signature Captured
field, select the reason why you were not able to obtain a
signature from this case participant.
Participated
in Plan
Select Yes or No
to indicate whether this case member participated in the plan.
Agreed
with Plan
If you selected
Yes in the Participated in
Plan field, select Yes or No to indicate whether the
case member agreed with the plan.
Date
copy of plan provided...
Enter the date a
copy of the plan was provided to the parent, guardian, custodian,
or guardian ad litem.

you selected above did not participate in the plan, explain
why. You can enter up to 250 characters.
