Child study inventory details screen

This screen allows you to view, add, or update the details of a child study inventory (CSI) facesheet. A child study must be completed periodically for every adoptive child, according to the practices of your agency. Once a study is completed, you record a facesheet in Ohio SACWIS. The facesheet provides a convenient listing of the tasks that need to be completed. It is used to record or verify components of the child’s case that are provided to the adoptive family at or before the adoption placement.

The Print Form link at the bottom of this screen allows you to print the JFS01685 report.

Fields on this screen

Initial Completion Date	Enter the date this facesheet is completed. Use the format MM/DD/YYYY. You cannot enter a date in the future. This field is required. A child's case can have only one ”r;initial” CSI facesheet record.
Reason for Update	If you are updating an existing facesheet record, select the reason. This field is required when you update a record. You can edit only the most recent CSI facesheet .
CSI Updated Date	If you are updating an existing facesheet record, enter the date. Use the format MM/DD/YYYY. You cannot enter a date in the future. This field is required for an update.
If Other	If you selected Other in the Reason for Update field, describe the reason. You can enter up to 200 characters.
Financial and Medical Resource Information Provided...	Select the check box next to each item that was provided to the adoptive parents. You can select more than one box. If you select Other, enter a detailed description of the item(s) in the text field below. You can enter up to 3,000 characters in that field.
Identified and Anticipated Special Needs of the Child	Enter a detailed description of the child's needs. You can enter up to 3,000 characters.
Checklist of Items to be Provided...	Select the check box next to each item that will be provided to the adoptive parents at placement. If you select Other, enter a detailed description of the item(s) in the text field below. You can enter up to 3,000 characters in that field.
