Select and Complete the Appropriate Case Circumstance

Select the level of safety threat  that applies to this case. This field is required.

Enter a detailed response to the instruction that appears above  the narrative field(s). These instructions appear according to your selection in the Select and Complete... field at the top of the screen. You can enter up to 4,000 characters in each narrative field.

Select each check box that describes the safety response review. The check boxes appear according to your selection in the Select and Complete... field at the top of the screen.

Select the radio button next to the safety response that applies.

Selecting Maintain indicates that, based on this safety review, the safety response has not changed since the last assessment of safety.

Selecting Create indicates that, based on this safety review, changes were identified in protective capacities or child vulnerability. These changes place the child(ren) in immediate danger of serious harm, necessitating the safety plan.

Selecting Modify indicates that, based on this safety review, an existing safety response must be modified. Changes have been made in safety threats, protective capacities, and/or child vulnerability, necessitating the change to an existing safety plan (including a legally authorized out-of-home placement)

Selecting Discontinue indicates that, based on this safety review,all threats resulting in the safety response have been controlled and/or resolved, or there has been adequate change in protective capacities or child vulnerability to protect the child(ren) from serious harm. The safety response is no longer needed.

Selecting Previously Discontinued indicates that, based on this safety review, a safety threat is not currently active but has been active at any time since the last assessment of safety, and the safety response was discontinued.
