Payment history search screen

This screen allows you to display a payment request history based on the payee, provider, or person. Each search criteria link opens the fields you can use for your search.

Fields on this screen

Agency	Select the agency whose payment requests you want to view if it is different from your own agency.
Payee	Click the Search Payee button, then locate and select the payee.
Payee #	If you do no wish to locate and select the payee from the Search Payee button, enter the identification number of the payee here.
Vendor #	If you do no wish to locate and select the payee from the Search Payee button, enter the payee's vendor number here.
Provider ID	If you do no wish to locate and select the provider from the Search Provider button, enter the identification number of the provider here.
Person	If you do no wish to locate and select the child from the Search Person button, enter the child's name here.
Person ID	If you do no wish to locate and select the child from the Search Person button, enter the child's person identification number here.
Service Category	Select the service category of the payment request to include it in your search criteria.
Auth #	Enter the authorization number of the payment request to include it in your search criteria.
Service	Select the service of the payment request to include it in your search criteria.
Warrant #	Enter the warrant number on the payment request to include it in your search criteria.
Contract #	Enter the provider's contract number to include it in your search criteria.
Warrant Date	Enter the date of the warrant to include it in your search criteria. Use the format MM/DD/YYYY.
Invoice #	Enter the provider's invoice number to include it in your search criteria.
Warrant Type	Select the type of warrant to include it in your search criteria.
Case #	Enter the case number on the payment request to include it in your search criteria.
Claim Date, To	To include claim date range in your search criteria, enter the beginning date of the range in the first date field and the ending date of the range in the second date field.
Disburse Date, To	To include disbursement date range in your search criteria, enter the beginning date of the range in the first date field and the ending date of the range in the second date field.
Disburse Name	Enter the name of the disbursement to include it in your search criteria.
