This screen allows you to view, add, or update a record of the child's Medicaid application. The system automatically creates a Medicaid record when eligibility has been determined and a reimbursability record has been created.

Application Type	Select the type of application. This field is required.
Medicaid Application Date	Enter the date of the child's application for Medicaid benefits. Use the format MM/DD/YYYY. This field is required.
Requested Effective Date	Enter the date on which Medicaid benefits were requested to begin. Use the format MM/DD/YYYY. This field is required.
Medicaid Type	Select the type of Medicaid services the child applied for. This field is required.
Medicaid Effective Date	This field reflects the date the child was eligible to receive Medicaid benefits, according to information in MMIS.
Medicaid Termination Date	This field reflects the date the child's Medicaid benefits terminated, according to information in MMIS.
Status	Select the status of the Medicaid application record. This field is required.
Comments	Enter any additional information about the child's Medicaid application. You can enter up to 4000 characters.
