View Medicaid Card History

Historical details regarding a person's Medicaid card can be viewed within SACWIS.

View Medicaid card history as follows:

1. Click the Financial tab at the top of the screen.

2. Click the Eligibility tab.

3. Click the Medicaid Card History link in the left navigation bar. The Search Criteria screen appears.

4. Select Search Type of by title IV-E number or by SACWIS person ID.

5. Enter the number associated with the search type in the Search Key field.

6. Click Search. the results appear in the Search Results table.  

7. Click Home, another tab or link to leave this screen.
