View a Person's SACWIS History

Ohio SACWIS provides a history of a person's case, intake, provider, and protective services alert (PSA) records. To view these histories, complete the following steps:

1. Conduct a Person Search.

   1. From the Person Search Criteria screen, enter the criteria that you want to use for the search.

   2. Click the Advanced Search Criteria link if you want to search using a social security number, reference number, or address.

   3. Click the Search button. The results will display in the Person Search Results table, or you will see, No Results Returned.

2. From the Person Overview screen, click the SACWIS History link in the navigation bar on the left. The SACWIS History screen appears.

3. Click the    next to the appropriate history link, or click on the link name. Each link shows a table listing all of the person's records within that category.
   
   The View Original Person link displays the original person profile of an adopted child. It is available only to workers whose security profile allows them access to this information.

4. Within the various SACWIS History tables links; additional links may appear where you can view details about those particular records as needed.

Example: If you click on the Case Member History link, a link appears in the Case ID field. Click the Case ID link to view details about that case record.

1. Click the Close button to return to the Person Overview screen.
