Generate and Print an Intake Report Worksheet Form

The information entered on an intake may be generated, and printed as a report. To generate, and/or print an intake worksheet form, a category must be selected and saved to the record. There are two report parameter options in which to display and print the report: with or without reporter information; and/or with or without SDM comments. The report generated will display a Draft watermark until the intake decision is saved.

Generate an intake report as follows:

  1. Click the Intake tab. The Intake Workload screen appears. The system defaults to display all intakes currently being processed based on your role.

  2. Click in the View By column, and select a filter option from the drop down list.

  3. Click Filter. The Intake Workload lists the intakes based on the filtered view.

You can sort these three columns in ascending or descending order to help locate the intake:  

Date/Time Received

Screener Name/SDM Name

Status

  1. Once the intake is located; click on the icon in the far right column of the intake. The Document Details screen appears. It will display any saved intake reports as a link in the Document History section.

  2. Click Generate Report. The Intake Report Worksheet Form screen appears.

  3. In the Print form with Reporter Information section, select No if you do not want the report to include reporter information.  

  4. In the Print form with SDM Comments section, select No if you do not want the report to include SDM comments.

  5. Click Generate Report. Cancel will stop the report from generating and return you to the intake workload.

The report will open and display based on the information entered, and the report parameters selected.

At the bottom of the report, click Save to save the report to the system. It will then display on the Document Details screen in the Document History section as a link when you go to print another intake. Click Cancel to close the report generated and return to the intake workload. Click Review Parameters if you want the report generated using different options.

If a DRAFT watermark displays on the report, the intake decision has not been saved. An intake decision must be saved to remove the DRAFT watermark.

  1. To print the report, hover your cursor in the body of the report to display the menu: .

Or, you can click the Esc key on your PC keyboard to display the report in PDF format. Options will display:      

  1. Click the printer icon, and set up the desired print parameters.  

  2. Click the Print button.
