End-Date a Provider Member

If a household or staff member of a non-ODJFS provider leaves the provider's household, you end-date the member's record. The member remains in the provider's member history. (You cannot delete or remove a member from the provider history.)

End-date a provider member as follows:

  1. Click the Provider tab at the top of the screen.

  2. Click the Workload tab. The Workload screen appears.

  3. Click the Select link for the provider whose name you want to update. The Provider Overview screen appears.

  4. Click the Provider Information link in the Provider Actions field. The system defaults to the Basic tab; the Provider Name Information screen appears. The Basic tab displays a history of the provider's names, addresses, and statuses.

  5. Click the Members tab. The Current Active Members screen appears. This tab lists the current household or staff members. It also displays the provider's marital status.

  6. Click the Edit link for the provider member you are end-dating. The Member Details screen appears.

  7. Complete the End Date and End Date Reason fields.

  8. Click OK. You are returned to the Members tab.

  9. Click Save. You are returned to the Provider Overview screen.
