Record a BCII or FBI Check for a Provider

A BCII or FBI check on a provider, or prospective provider, is record both as an activity log entry and as a verification on the home study. The OAC rule requires you to create a separate BCII or FBI record check for each household member. The activity log entry allows you to associate the check with a particular member. On the verification record, you can identify the member for whom a check was completed in the narrative field.

Record a BCII or FBI check as an activity log entry as follows:

Open the provider's record.
From the Provider Overview screen, click the Activity Log link in the menu on the left. The Activity Log Filter Criteria screen appears.
Click Add Activity. The Activity Details screen appears.
Select the Responsible Worker field.
Enter the date you complete the check in the Activity Start Date field.
Select General in the Contact Type field.
Select Foster Home Applicant, Adoptive Home Applicant, or Kinship Care Applicant in the Category field.
Select BCII Check or FBI Check in the Sub-Category field.
Click the Associate Member link in the Activity Association area. The Associate Members screen appears.
Select the check mark next to the member on whom you completed the check. Click only one member.
Click OK. You are returned to the Activity Details screen.
In the Narrative field, enter the following details:
- Date the card was mailed to BCII
- Response – Prohibitive, Not prohibitive, or No criminal record
- Rehabilitated – Yes, No, or Not applicable (i.e., no offense)
- Whether the person meets the 5-year residency in Ohio rule – Yes or No
When the activity log record is complete, select Completed in the Activity State field.
Click Save. You are returned to the Provider Overview screen.

Record a BCII or FBI check as a verification on the home study record as follows. These are each separate verifications.

From the Home screen, click the Provider tab.
Click the Workload tab. The Provider Workload screen appears.
Click the Select link for the provider record you want to update. The Provider Overview screen appears.
Click the Home Study link in the menu on the left. The Maintain Home Study History screen appears.
Click the View link for the home study record.
Click the Verifications link in the Topic column. The Maintain Verification Tasks screen appears.
Click the Edit link next to Criminal Record Check or FBI Check. The Verification Details screen appears.
Complete the fields on this screen. In the Narrative field, enter the following details:
- Member the check pertains to
- Date the card was mailed to BCII
- Response – Prohibitive, Not prohibitive, or No criminal record
- Rehabilitated – Yes, No, or Not applicable (i.e., no offense)

Click Save. You are returned to the Verification Details screen.
Click Close. You are returned to the Maintain Home Study Information screen.
Click Close. You are returned to the Maintain Home Study History screen.
