Record the Suspension or Revocation of a Worker's License

If a worker's professional license was suspended or revoked, this fact must be recorded in Ohio SACWIS.

To record a suspension or revocation of a worker's license, complete the following steps:

  1. From the Home screen, click the Administration tab.

  2. Click the Staff tab.

  3. Click the Maintain Staff link in the menu on the light blue menu on the left. The Employee Search Criteria screen appears.

  4. Enter search criteria. The quickest way to locate the worker's record is to enter the Employee ID number. If you don't have that, you can search by the worker's name, education level, and so on.

  5. Click the Search button. The worker records that match your search criteria appear in the search results table.

  6. Click the Edit link next to the worker's name. The system displays the worker's record on the Employee Information screen.

  7. Click the Licenses tab. The Professional Licenses list screen appears.

  8. Click the Edit link for the license that was suspended or revoked. The License Details screen appears.

  9. For a suspension, select the check box next to Suspended, then enter the dates during which the license is suspended.

    For a revocation, select the check box next to Revoked, then enter the date on which the license was revoked.

  10. If needed, enter a further explanation in the Comments field.

  11. Click the Save button. You are returned to the Professional Licenses list screen.

  12. Click the Save button again. You are returned to the Employee Search Criteria screen.
