Record an Independent Living Plan

The agency provides independent living services for emancipated youth to achieve their goals for self sufficiency. You record an independent living plan for all youth in the custody of the agency who are 16 years of age and older. The plan documents any concerns and services planned or provided to help the youth become self sufficient.

An independent living plan may be edited if it is in "Pending" status, which describes a plan that has not yet begun. Once the youth has begun the activities to meet the goals stated in the plan, you update the status to "Active." You can change the status to "Active" or "Complete" when at least one independent living goal has been created for each requirement topic on the goals screen. Once the plan is in "Active" status, you cannot edit the basic plan information.

Process

Record an independent living plan as follows:

  1. From the Home screen, click the Case tab.

  2. Click the Workload tab. The Case Workload screen appears.

  3. Click the case ID Number link of the case you want to update. The Case Overview screen appears.

  4. Click the Independent Living link in the navigation bar on the left. The Independent Living Records screen appears. The screen lists any independent living plans that have been recorded for the youth on this case.

  5. Click the Edit link next to the name of the youth whose plan you want to update, or select the youth's name in the Child Name field, then click Add Independent Living to create a new plan.

  6. Record basic independent living plan details.

  7. Record independent living plan goals and requirements.

  8. Record an independent living plan readiness review.

  9. Record the completion of an independent living plan.
