Record or edit concerns in AR family service plan

Process: Record or Edit an AR Family Service Plan

An AR family service plan should be completed within 30 days of an AR pathway switch.

Record concerns in an AR family service plan as follows:

1. From the AR Family Service Plan Filter Criteria screen; click Edit next to the service plan you want to add/edit concerns. The Identifying Information screen appears.

2. Select the Concerns link. The Concerns screen appears.

3. Click the Add Concerns button. The Concerns Details screen appears.

4. Select the Case Members. Answer the questions to complete each of the Narrative fields.  

5. Click Save. You are returned to the Concerns Details screen.

6. Click Close. You are returned to the Identifying Information screen.

7. Click next step below.
