Record Family Concerns

Process: Record a Case Plan

The concerns (risk contributors) identify the needs the case plan should address in helping the family. A concern record must be associated with a service record. The case plan must include at least one strength and one concern.

The system automatically fills in the family concerns recorded on the family assessment. You can update these on the case plan as necessary.

Record family concerns as follows:

  1. From the Case Plan Topics screen; select the Strengths and Concerns link. Select the Concerns tab. The Concerns screen appears. It lists the concerns (risk contributors) that were entered on the family assessment.

  2. Click the Edit link next to the participant names to update an existing concern record, or click Add Concern to add a new concern record. The Concerns Details screen appears. It lists the case members who have been added to the plan as participants.

  3. Select the check box next to the name of each participant to whom the concern record applies.

  4. Select the behavioral category(ies) in the Available Risk Contributors field, then click Add>>. The category(ies) you selected appear in the Selected Risk Contributors field on the right.

  5. Enter a detailed response in the What is the concern? narrative field, then complete any other narrative fields that apply.

    You record the expected changes or services to document the behaviors, activities, services that will assist in the reduction of risk.

  6. Click Save. You are returned to the Concerns screen. The concern record you just added appears in the table.

  7. Click the Services link to associate this concern with a service that will help reduce risk to the child. The Service Details screen appears. Each concern must be associated with a service record.

  8. Click the Link Services button. The Case Services Filter Criteria screen appears.

  9. Enter the information to filter the services for selection. Click the Select link for the service you want to associate with this case plan. (You cannot add a new case service from the case plan record.)

    The system determines whether service authorization is necessary. If this is a paid service, the system displays the Authorize link next to the case plan on the Case Plan Filter Criteria screen for the record once the status is updated to "Provided."

  10. Once you have returned to the Concerns screen, click Close. You are returned to the Case Plan Topics screen.

  11. Click Next Task below to continue.
