Record Placement Information (Case Plan)

Process: Record a Case Plan

The case plan asks you to address removal setting and placement information when one or more children are currently in placement. (If this does not apply to your case plan, click Next Task below to continue.)

The system automatically fills in information from the most current placement record.

Removal setting information is recorded in the case plan for all children for whom the PCSA maintains custodial rights. Removal information is recorded to document how the selected placement will meet the needs of  the child.

Record placement information as follows:

  1. From the Case Plan Topics screen; select the Placement Information link. The Settings and Placement screen appears.

    The system displays the child's current placement setting for each child who is a participant in the case plan, as entered on the child's placement record. If the child is not in placement, the system displays "own home."  

    This information reflects why each less restrictive placement setting was not selected. Only less restrictive placement settings will appear. (For example, if you selected Certified Foster Home as the placement type, you address only these setting types: Own Home, Certified/Approved Relative Home, and  Certified/Approved Non-Relative Home.)

  2. Click the Edit link next to the setting description. The Narrative Details screen appears.

  3. Enter a detailed description of the setting in the Describe field.

  4. Click Save. You are returned to the Settings and Placement screen.

  5. Repeat steps 2 - 4 for each child on this screen.

  6. Click the Placement tab. The Placement Details screen appears. It displays a series of questions asking for details about the placement setting identified for each child in the case plan.

  7. Click the Edit link next to each question. The Narrative Details screen appears.

  8. Enter a detailed description of the setting in the Describe field.

  9. Click Save. You are returned to the Settings and Placement screen.

  10. Repeat steps 7 - 9 for each child on this screen.

  11. Click Close. You are returned to the Case Plan Topics screen.

  12. Click Next Task below to continue.
