Court Case Number

Enter the number of the court case to which this subpoena relates, if any.

Subpoena Type

Select the type of subpoena(s) the person received, then click Add>>.  You can select more than one type. This field is required.

Subpoenaed Person

Click Person Search, then locate and select the person who received the subpoena. This field is required unless the subpoena is sent or received for records.

Issuing Court

Enter the name of the court that issued the subpoena. You can enter up to 60 characters.

Appearance Location

Enter an address or describe the location where the person is required to appear. You can enter up to 250 characters.

Date Subpoena Served

Enter the date on which the person received the subpoena. Use the format MM/DD/YYYY. You cannot enter a date in the future.

Time Subpoena Served

Enter the time when the person received the subpoena. Use the format HH:MM, then select AM or PM.

Date Ordered to Appear

Enter the date on which the person is required to appear. Use the format MM/DD/YYYY.

Time Ordered to Appear

Enter the time when the person is required to appear. Use the format HH:MM, then select AM or PM.

File Stamped

Enter the date on which the court file-stamped the subpoena. Use the format MM/DD/YYYY. You cannot enter a date in the future.

Subpoena Reason

Describe why the person was subpoenaed. You can enter up to 3,000 characters.

Agency Response

Select the response or action from the agency.

Response Date

Enter the date on which the agency responded. Use the format MM/DD/YYYY.

Other

If you selected Other in the previous field, describe the agency's response here. You can enter up to 500 characters.

Subpoena Record has been Created in Error

Select this check box if this subpoena was created in error and you want to invalidate it. (You cannot delete a subpoena record.)
