Hearing Date

Enter the date the hearing took place. Use the format MM/DD/YYYY. This field is required.

Hearing Time

Enter the time the hearing began. Use the format HH:MM, then select AM or PM. This field is required.

Hearing Type

Select the type of hearing.

Court Case Number

Enter the court case number. This is a court-assigned number that includes all children in a family.

Court ID Number

Enter the court ID number. This is a court-assigned, unique number given to each child in a family.

Court Name

Enter the name of the court, for example, the county name and the court type. You can enter up to 40 characters.
