Adjudication/disposition details

Adjudication Date

Enter the date on which the adjudication was made. Use the format MM/DD/YYYY. You must enter either this date or a disposition date.

Adjudication Type

Select the adjudication type.

Disposition Date

Enter the date of the disposition. Use the format MM/DD/YYYY. You must enter either this date or an adjudication date.

Offense Type

Select the offense, then click Add>> . The offense you added appears on the right. You may select more than one offense type.

Court Case Number

Enter the court case number. This is a court-assigned number that applies to all children in the family.

Court ID Number

Enter the court ID number. This is a court-assigned number that applies to the specific child.

Court Name

Enter the court name, for example, the county and court type.

Violent Offender

Select the check boxes that apply if the child is adjudicated a violent offender.

Sexual Offender

Select the check boxes that apply if the child is adjudicated a sexual offender.

Registered

Select this box if the child has been registered as a sexual offender, then enter the date of this registration in the Date field. Use the format MM/DD/YYYY.

Probation

This box is checked from the dispositional portion of a hearing.

Adjudication/ Disposition Narrative

Enter a summary of the adjudication or disposition. You can enter up to 3,000 characters.

Adjudication Record Created in Error

Select this check box if you created this record by mistake. (You cannot delete a court record.)
