Removal information

Child Name

This field displays the child selected on the Initial Removal Record screen.

Current Legal Status

Select the current legal status for the child who was removed.  This field is required.

Circumstance

Select the circumstance of the removal. This field is required.

Responsible School District

Select the school district in which the parents, guardians, or custodians reside at the time of the child’s removal. This field is required.

Date Removed

Enter the date on which the child was removed from the home. Use the format MM/DD/YYYY. This field is required.

Primary Caretaker

Select the person on the case who takes primary responsibility for the child's care at home. This field is required.

Secondary Caretaker

Select the person on the case who takes secondary responsibility for the child's care at home.

Caretaker Structure

Select the living arrangement from which the child was removed.

Available Removal Reason(s)

Select the reason(s) why the child was removed from home, then click Add>>. You may select more than one reason.

Selected Removal Reason(s)

The reason(s) you selected for the child's removal from home appear here. To remove a reason, select it, then click <<Remove.

Discharge Date

The system displays the discharge date from the Placement Ending Details screen when the reason selected is "Discharge."

Discharge Reason

Select the reason why the child was discharged.

Status

Select the status of this removal record. This field is required.
