Date of Pre-Adoptive Staffing	This field displays the date that was entered on the initial screen. This is the date on which the initial pre-adoptive staffing conference is or was held. Enter a different date if necessary, for example, if the meeting was held on a different day than was originally planned. Use the format MM/DD/YYYY. This field is required.
Attended Staffing	Select this check box if the participant attended the pre-adoptive staffing conference.
