This screen is the first tab of the independent plan. The agency provides independent living services to age-appropriate youth in agency custody and for emancipated youth. An independent living plan helps the youth reach goals for self sufficiency.
It is where you record dates and details with regard to type of independent living assessment, identified concerns, services planned, and so on. It also provides statistical data for reports and Chafee requirements for independent living youth.
On this screen of the IL plan, you will record assessment information. An assessment is usually recorded before the plan is completed. You will also record IL plan information details. Only one plan may be in effect for a child at a given time.
All fields can be edited while the plan is in Pending status. Once the plan status is saved as Active, date fields, and the assessment tool used are grayed out and cannot be edited. After the plan is saved with the status of Closed, the record becomes view only. No details can be changed.
