Independent living screen

This screen is the first tab of the independent plan. The agency provides independent living services to age-appropriate youth in agency custody and for emancipated youth. An independent living plan helps the youth reach goals for self sufficiency.

It is where you record dates and details with regard to type of independent living assessment, identified concerns, services planned, and so on. It also provides statistical data for reports and Chafee requirements for independent living youth.

On this screen of the IL plan, you will record assessment information. An assessment is usually recorded before the plan is completed. You will also record IL plan information details. Only one plan may be in effect for a child at a given time.

All fields can be edited while the plan is in Pending status. Once the plan status is saved as Active, date fields, and the assessment tool used are grayed out and cannot be edited. After the plan is saved with the status of Closed, the record becomes view only. No details can be changed.

Fields on this screen

Effective Date	Enter the date on which this independent plan is in effect. Use the format MM/DD/YYYY. This field is required. You cannot enter a date in the future. This date must be later than the end date of the previous plan. You cannot change this date if the plan status is "Active."
Status	Select the status of the independent living plan. The field defaults to "Pending." This field is required. You cannot select "Active" or "Closed" unless at least one requirements record has been added to the plan. Once you change the status to "Active," you cannot change these fields: Effective Date, Youth Information, Date Referred/ Emancipated, and Anticipated Emancipation Date. You cannot change the status from "Active" to "Pending" after you save the record.
End Date	Enter the date on which this independent living plan ends. Use the format MM/DD/YYYY. This field is required if you select a status of "Closed." This date must be the same as or later than the effective date. You cannot update this plan once you have saved the end date.
Youth Information	Select the youth's status, which describes the reason for the independent living plan. This field is required. You cannot change this selection once the plan status is "Active."
Date Referred/Emancipated	Enter the date the youth was either referred for an independent living plan or emancipated. Use the format MM/DD/YYYY. This field is required. You cannot change this date once the plan status is "Active."
Anticipated Emancipation Date	If the youth is not already emancipated, enter the date when you expect the youth to be emancipated. You cannot change this date once the plan status is "Active."
IL Skills Assessment Completed	Indicate whether a skills assessment for independent living has been completed. This field is required.
Assessment Tool Used	Enter the name of the tool used in the skills assessment. You can enter up to 30 characters.
Date Assessment Completed	Enter the date on which the skills assessment was completed. Use the format MM/DD/YYYY.
IL Plan Completed	Indicate whether this independent living plan was completed. This field is required. Once you enter this date, you cannot edit any requirements records.
Date Plan Completed	Enter the date on which this independent living plan was completed. Use the format MM/DD/YYYY. This field is used when the status of the plan is "Complete."
(Input statements)	For each statement, select the activity that applies. Also enter or select the name as it applies. These statements are all required.
Youth is Currently Receiving IL Services/Training	Indicate whether independent living plan services or training are being provided to this youth.
