SAMS subsidy screen

This screen allows you to view or add a State Adoption Maintenance Subsidy. You can enter a request for a SAMS only when an request for Adoption Assistance (AA) has been entered in the system and denied. You cannot enter a SAMS request when a pending or approved AA or SAMS subsidy request already exists.

The Special Needs Criteria link displays a screen on which you view or select the special needs factors that apply to this child. The Calculate button instructs the system to determine whether the family's income exceeds 120% of Ohio's estimated median income. The Determine Eligibility button instructs the system to determine whether the child is eligible. When the system determines a child's eligibility, it checks to see that certain criteria are met.

Payments under this type of subsidy are managed in the system as a recurring payment under the service authorization process.

Application Received Date	Enter the date the agency received the family's application for a SAMS. Use the format MM/DD/YYYY.
Home Study Date	If the parent(s) is a non-ODJFS (out-of-state) provider, enter the date on which the parent's home study was approved. Use the format MM/DD/YYYY. This field is required. The system automatically fills in this date for an ODJFS adoptive care provider.
SAMS Approval Date	Enter the date on which the SAMS agreement was approved. This field is enabled when the system determines that the child is eligible for the subsidy.
Social/Med Date	Enter the date //SME: what is this?// Use the format MM/DD/YYYY. This field is required.
Subsidy Effective Date	Enter the date on which the SAMS agreement was completed. This field is enabled when the system determines that the child is eligible for the subsidy.
Annual Family Gross Income	Enter the family's gross income. This amount is used when the system determines whether the family's income is 120% of Ohio's estimated median income.
Family Size Including Adoptive Children	Enter the total number of children in the family. This number is used when the system determines whether the family's income is 120% of Ohio's estimated median income.
Subsidy Amount	If the child is eligible for the SAMS, enter the amount of the subsidy.
