Intake ID	Select the intake that applies to this law enforcement involvement record. This list displays the intakes that are currently linked to this case.
Date Requested	Enter the date on which the worker requested law enforcement assistance. Use the format MM/DD/YYYY. This field is required.
Time	Enter the time law enforcement arrived, then select AM or PM. Use the format HH:MM.
Jurisdiction	Enter the legal jurisdiction this case falls under. You can enter up to 60 characters. This field is required.
Response Date	Enter the date on which law enforcement responded to the worker's request for assistance. Use the format MM/DD/YYYY.
Lead Officer	Enter the name of the lead officer who responded. You can enter up to 60 characters.
Badge No.	Enter the lead officer's badge number. You can enter up to 10 characters.
Additional Officers Assigned	Enter the names of any other offices who responded. You can enter up to 300 characters.
Law Enforcement Assisted	Select Yes or No to indicate whether law enforcement responded and assisted in the situation.
If No, Explain	If you selected No in the previous field, explain why law enforcement did not assist.
Results	Explain the results of law enforcement's involvement in this situation. You can enter up to 2,000 characters.
