Maintain safety plan authorizations screen

This screen allows you to enter an authorization for this safety plan and indicate whether you have obtained signatures from the adult participants.

The Validate for Approval button allows you to ensure that you've entered all information that's required. It opens the Validation Messages screen, which lists the information, if any, that needs to be completed before you can submit this work item for approval.

Safety Plan Created as a Result of	Select the reason for creating this safety plan.
ID	Enter the ID number of the work item you selected in the Safety Plan Created as a Result of field.
Has the parent/ custodian/ guardian(s) read or been read...	Select Yes or No in response to this question.
Has the parent/ custodian/ guardian(s) provided verbal approval...	Select Yes or No in response to this question.
Date Provided	If you selected Yes to the previous question, enter the date on which the parent/custodian provided approval. Use the format MM/DD/YYYY.
Time Provided	If you selected Yes to the previous question, enter the date on which the parent/custodian provided approval. Use the format MM/DD/YYYY.
Signature Received	Select Yes or No to indicate whether the agency received the parent/custodian's signature on the safety plan.
Date Provided	Enter the date a signature was received. Use the format MM/DD/YYYY.
