Date the Agency Contacted the Out-of-Home Care Administrator	Enter the date a worker from your agency contacted the out-of-home care administrator of the allegations. Use the forma MM/DD/YYYY.
Time	Enter the time the notification was made. Use the format HH:MM.
Person Notified	Enter the first and last names of the out-of-home care administrator. You can enter up to 50 characters.
How Notified	Select the method by which your agency notified the out-of-home care administrator.

Date Agency Provided Written Notification of Allegations to Out-of-Home Care Administrator	Enter the date a worker at your agency provided the out-of-home care administrator with the allegations. Use the format MM/DD/YYYY.
Date the Licensing Agency/ Supervising Authority Notified of Receipt of the Report	Enter the date the agency or authority was notified of the report. Use the format MM/DD/YYYY.
Time	Enter the time the agency or authority was notified. Use the format HH:MM.
Person Notified	Enter the first and last names of the person at the licensing authority or supervising agency who was notified. You can enter up to 50 characters.
How Notified	Select the method by which your agency notified the agency or authority.
Is this a Third Party Investigation?	Select Yes or No to answer the question.
Date Law Enforcement Contacted for a Third Party Investigation	If you selected Yes to the field above, enter the date on which a worker at your agency contacted law enforcement. Use the format MM/DD/YYYY.
Time	Enter the time law enforcement was notified. Use the format HH:MM.
Person Notified	Enter the first and last names of the person at the law enforcement agency who was notified. You can enter up to 50 characters.
How Notified	Select the method by which the person in the previous field was notified.
Date Agency Sent Written Request to Law Enforcement	Enter the date on which a worker at your agency sent a request to a law enforcement agency. Use the format MM/DD/YYYY.
