ODJFS response details screen

As the home study assessor, you use this screen to view the response from ODJFS to your recommendation that the provider not be approved or certified because of rule violations.

As the agency or ODJFS reviewer, you add the required response from this screen. The system provides you, as the reviewer, with a few options. You can accept the assessor's recommendation and approve or disapprove the provider. You can reject the recommendation, then on the Decision tab still decide to revoke the license because of other rule violations. Or you can accept the rule violations but decide not to revoke the license. You may also update the waivers, variances, or rule violations if this record is in "Review" status.

Specific Rule Response	Select this radio button if this response addresses a specific rule the provider violated.
General Response to Rules	Select this radio button if this response addresses the provider's compliance with rules in general.
Rule Chapter	If the provider violated a specific rule, select the rule chapter.
Section	If the provider violated a specific rule, select the section.
Subsection	If the provider violated a specific rule, select the subsection
Decision	Select the decision.
Effective Date	Enter the date the decision was in effect. Use the format MM/DD/YYYY. This field is required.
Explain	Enter any additional information about this response. You can enter up to 2,000 characters.
