Agency waiver request and recommendation details screen

In certain circumstances, an agency may request on behalf of a provider a waiver or variance from ODJFS rules. This screen allows you to view or update the details of such a request.

A waiver allows the provider to maintain certification status while the provider takes specific actions to comply with a State rule within a certain time frame. A waiver request may be renewed once. A variance allows the provider to disregard a certain State rule.

After a recommendation for a waiver or variance is recorded, it is reviewed by an ODJFS worker. Once an ODJFS worker has recorded a response to the recommendation, the record cannot be updated.

Fields on this screen

Initiated By	Select the worker who initiated this waiver or variance request. This field displays all workers who are currently assigned to this provider. This field is required.
Type	Select the type of waiver or variance.
State of Waiver/ Variance	Select the state of the waiver, whether it is an initial waiver or a renewal. This field is disabled for a variance request.
Effective Date	Enter the date this waiver or variance request is in effect. Use the format MM/DD/YYYY. This date cannot be earlier than the effective date on the certification to which this waiver or variance is linked.
Expiration Date	Enter the date this waiver or variance request expires. Use the format MM/DD/YYYY. This date cannot be later than the expiration date on the certification record to which this waiver is linked. This field is disabled for a variance request. When a waiver expires, the system sends a notification to the worker assigned to this provider.
Rule Chapter	Select the chapter of the rule from which you are requesting a waiver or variance. This field is required.
Rule Section	Select the rule from which you are requesting a waiver or variance. This field is required.
Rule Sub-Section	Select the subsection of the rule from which you are requesting a waiver or variance.
Explain	Enter any additional information about this waiver or variance request. You can enter up to 2000 characters.
