Record a Provider Waiver Request

In certain circumstances, an agency may request on behalf of a provider a waiver from ODJFS rules. A waiver allows the provider to maintain certification status while the provider takes specific actions to comply with a State rule within a certain time frame. A waiver request may be renewed once.

A waiver request may be made during the certification process. (See View a recommendation for certification or Link a recommendation to a waiver request.) Or if the provider is currently certified, you can request a waiver at any time.

Record a waiver request as follows:

  1. From the Home screen, click the Provider tab.

  2. Click the Workload tab. The Workload screen appears.

  3. Click the Select link next to the ID of the provider whose recommendation you want to record. The Provider Overview screen appears.

  4. Click the Waiver link in the navigation bar on the left. The Maintain Waiver Requests screen appears. It lists a history of this provider's waiver records. A view link appears when the reviewer has completed a decision. A Edit link appears until a decision has been entered.

  5. Click the Edit link to update an existing record that is in Pending status; or Add Request to add a new request. The Agency Waiver Request and Recommendation Details screen appears.

  6. Complete the fields on this screen. (The review completes the Explain field.)

  7. Click Save. You are returned to the Maintain Waiver Requests.

  8. Click the Edit link. The Agency Waiver Request and Recommendation Details screen appears.

  9. Click the Process Approval link to complete the process for review and final approval.  

    The reviewer then records his or her decision.
