Contract amendment details screen

This screen allows you to record an amendment to the contract you selected on the Contract Details screen. You can amend a contract to change the original dates or amount. The system automatically displays the amendment number. A contract may be amended only if the status is "Active."

Fields on this screen

Amendment Number	The system automatically determines this number. You cannot edit this field.
Amendment Approval Date	Enter the date on which the amendment was approved. Use the format MM/DD/YYYY.
Amended Begin	Enter the effective date of this amendment. This date must be the same as or later than the original begin date if this is the first amendment record. If this is a subsequent amendment record, this date must be later than the end date of the previous amendment record. Use the format MM/DD/YYYY. This field is required.
Amended End	Enter the new ending date of this amended contract. Use the format MM/DD/YYYY. If this date extends the original contract end date, the system will present a message asking you to confirm this action. If you respond Yes, the system extends the end date of the active services associated with this contract. If you respond No, the system modifies the contract end date only.
Amended Amount	If you are amending the amount of the contract, enter the new amount.
Comments	Enter any additional information about the amendment. You can enter up to 4,000 characters.
