Fields on this screen

Service Description	Select the type of placement service. The system displays only active placement services.
Specific Child	Click Person Search, then locate and select the child who will receive placement services under this contract.
Cost Begin	Enter the date when the provider can begin assessing this service cost. This date cannot be earlier than the contract begin date. This field is required.
Cost End	Enter the date when the provider can no longer assess this service cost. This date cannot be later than the end date of the contract. This field is required.
