Enter the beginning date of reporting period. Use the format MM/DD/YYYY. This field is required.

To Date

Enter the ending date of the reporting period. Use the format MM/DD/YYYY. This field is required.

Specific Age Range to

Enter the oldest age of children to be included in the report.

Agencies

Select the agency(ies) to be included in this report, then click Add>. The agency(ies) you select appear in the Selected list box. This field is required.

Report Type

Select the type of report you are generating.
