Available Agencies |
Select the agency(ies) whose intakes you want to include in the report, then click Add>>. |

This field displays the agency(ies) selected from the previous field. There must be at least one agency listed here before the report can be generated. |
Report Type |
Select the type of report you want to generate. |
From Date |
Enter the beginning date of the range when the intakes met your parameters. Use the format MM/DD/YYYY. This field is required. |
To Date |
Enter the ending date of the range when the intakes met your parameters. Use the format MM/DD/YYYY. This field is required. |
Time Received |
Enter the beginning time of intakes to include in the report, then select AM or PM. Use the format HH:MM. |
To |
Enter the ending time of intakes to include in the report, then select AM or PM. Use the format HH:MM |
Day Received |
Select the day of the week to include intakes received on a certain day. |
Intake Category |
Select the category of intake to include in the report. All categories are included if this field is left blank. |
Intake Type |
Select the type of category of intake to include in the report. All types are included if this field is left blank. |
Intake Status |
Select the state of intakes to include in the report. Intakes in any status are included if this field is left blank. |
Emergency Priority |
Select the intake priority to include in the report. Intakes of any priority are included if this field is left blank. |
County Priority |
Select the county priority to include in the report. Intakes of any priority are included if this field is left blank. |
Reporter Type |
Select the reporter type to include in the report. Intakes with any reporter type are include if this field is left blank. |
Race of ACV |
Select the race of alleged child victims to include in the report. Children of any race are included if this field is left blank. |
Age of ACV |
Select the beginning age of the alleged child victims to include in the report. Children of any age are included if the age fields are left blank.. |
To |
Select the ending age of the alleged child victims to include in the report. Children of any age are included if the age fields are left blank. |
Zip Code Type |
Select the type of zip code to include in the report. |
Available Zip Codes |
Select the zip code(s) of intakes to include in the report, then click Add>>. |
