Agency Type

Select the type of agency for which you are generating this report. This field is required.

Agency

Select the agency for which you are generating this report. This list displays the agencies of the type you selected above. This field is required.

Unit

Select the unit(s) to be included in this report, then click Add>. The list box displays the units in the agency you selected above. The unit(s) you selected appear in the Selected list box. This field is required.

Supervisor

Select the supervisor(s) to be included in this report, then click Add>. The list box displays the supervisors assigned to the unit(s) you selected above. The supervisor(s) you selected appear in the Selected list box. This field is required.

From Date

Enter the beginning date of the reporting period. Use the format MM/DD/YYYY.

Do Date

Enter the ending date of the reporting period. Use the format MM/DD/YYYY.
