Record Immunizations

Process: Record a medical profile

The medical profile includes the immunizations the child has received, or the reason why a child will not be immunized.

To record immunization records, conduct a Person Search. From the Person Overview screen, complete the following steps:

  1. Select the Medical from the left hand navigation. The Provider tab appears.

  2. Click the Immunization tab. The Immunization Information screen appears.

  3. If a decision was made not to immunize the child, select the Child will not be immunized check box.

  4. If the box is selected, enter an explanation for this in the Reason field.

  5. If the child has received any immunizations, click Add Immunization. Or click the Edit link to update an existing immunization record. The Immunization Information (detail) screen appears.

  6. Complete the Immunization Type and Date of Immunization fields. If you select Other for the type, enter a short description of it in the Other Immunization Type field.  

  7. Click the Save button. You are returned to the Immunization Information screen. The immunization record you added appears on the screen.

  8. Click Apply to stay in the Medical Profile. Click Save or Cancel to return to the Person Overview screen.

  9. Click Next Task below to continue.
