Through the home study process, a home provider, or ODJFS provider (see About provider information), completes the necessary tasks to become approved or certified by the agency. These include activities like provider training, assessment visits, home descriptions, references, and so on. These may be recorded in any order, as they are completed. However, all activities must completed within 6 months of the date the application was screened in (or sooner for an ICPC provider).
There are three types of home study records:
Initial. The home study record that initiated the provider's approval for the provider type and the level of care. You must complete an initial home study for each provider type record. If the provider moves to another level of care, a new initial home study is required.
Amended. The home study record created to reflect new information added after the initial home study record was approved. It ensures that the home study reflects the most current information. The home study assessor can change only certain information in an amended home study.
Updated. The home study record created during the recertification process.
