Close an Active Provider

When a home provider is no longer providing services to the agency, you record a transaction to close the approval/certification record and change the provider status.

Close an active provider as follows:

1. Close the provider's approval/certification

   1. Click the Provider tab at the top of the screen.

   2. Click the Workload tab. The Provider Workload screen appears.

   3. Click the Select link for the provider whose name you want to update. The Provider Overview screen appears.

   4. Click the Approval/Certification link in the menu on the left. The Maintain Transactions screen appears.

   5. Complete the following fields:
      
      Agency Contact Person
      Provider Type
      Transaction (Select Close)
      Recommendation Date
      Closed Reason

   6. Click Process for Approval. The Process Approval screen appears.

   7. Complete the following fields:
      
      Action (Select Approved and routed)
      Agency
      Reviewers/Approvers

   8. Click Save. The Maintain Transactions screen appears.

   9. Click Save. The Maintain Approval/Certification Recommendations list screen appears. The transaction you just created appears in the list.

2. Change the provider's status

   1. From the Provider Overview screen, click the Provider Information link. The Basic tab of the Provider Information screen appears.

   2. Select Add Status. The Provider Status Information screen appears.

   3. Complete these fields:
      
      Provider Status (Select Closed)
      Effective Date

   4. Click Save.

To re-initiate a close provider, you must create a new inquiry record. Once the inquiry record is complete, you can link it to the previously existing provider record.
