Ohio SACWIS allows you to keep track of the incidents when a provider violates an agency or ODJFS rule or when a complaint is filed against the provider. An investigation into the violation or complaint is conducted after you record the incident. (See Record a rule violation investigation and outcomes.)
Record a rule violations as follows:
From the Home screen, click the Provider tab.
Click the Workload tab. The Workload screen appears.
Click the Select link for the provider whose record you want to view. The Provider Overview screen appears.
Click the Rule Violations link in the navigation bar on the left. The Maintain Rule Violation Incidents screen appears. It lists each incident that has been recorded.
Click the Edit link for the pending incident you want to update, or click Add Rule Violation for a new incident. The system defaults to the Rule Violation tab; the Rule Violation Details screen appears. It lists any rule violations that have been recorded, as well as any CA/N incidents that have been linked to a rule violation. The CA/N incident record itself is managed through the Intake and Case areas. It is linked to the provider record if any provider member is involved in an allegation.
In the Incident Type field, select the incident that prompted the investigation into the rule violation.
If the rule violation is linked to a report of a CA/N incident, click Link CA/N Incidents. The CA/N Incidents screen appears. Select the check box next to the incident(s) that are related to this rule violation, then click OK. You are returned to the Rule Violation Details screen.
Click Add Rule Violation. The Alleged Rule Noncompliance screen appears.
Complete this screen to describe the alleged violation. You must complete at least the Rule Chapter field.
Click OK. You are returned to the Rule Violation Details screen.
If a complaint against the provider prompted this investigation, click the Complaint tab, then complete the fields on the Incident Details screen.
Click Save. You are returned to the Maintain Rule Violation Incidents screen.
