Record a Rule Violation Investigation and Outcomes

Ohio SACWIS allows you to keep track of the incidents when a provider violates an agency or ODJFS rule or when a complaint is filed against the provider. (See Record a rule violation.) Once the incident is record, an investigation into the incident is conducted. When the investigation is complete, you record a summary of the findings and the outcomes.

Record the investigation and outcomes as follows:

  1. From the Home screen, click the Provider tab.

  2. Click the Workload tab. The Workload screen appears.

  3. Click the Select link for the provider whose record you want to view. The Provider Overview screen appears.

  4. Click the Rule Violations link in the navigation bar on the left. The Maintain Rule Violation Incidents screen appears. It lists each incident that has been recorded.

  5. Click the Add Rule Violation button. Click the Edit link for the pending incident you want to update.  The system defaults to the Rules Violation tab; the Rule Violation Details screen appears.

  6. Click the Summary tab. The Summary Details screen appears.

  7. Enter a detailed description of your findings in the Explain field.

  8. Click the Outcome tab. The Outcome Details screen appears.

  9. Select the outcome from the Outcome Based on Rule Violation field. Complete the remaining fields that apply from the investigation.

  10. Click Save. You are returned to the Maintain Rule Violation Incidents screen.
