Process: Record a Case Plan
A permanency goal must be recorded on the case plan when the current legal status for one or more children in the case plan is "Permanent Custody" or "Permanent Planned Living Arrangement." (If this does not apply to your case plan, click Next Task below to continue.)
You record your efforts to establish permanent placements for all children in the custody of the agency who have a goal of adoption or planned permanent living arrangement. These efforts include the steps and activities taken to locate an adoptive home, relative home, legal guardian, or other permanent home.
Record permanency information as follows:
From the Case Plan Topics screen, click the Permanency Information link. The Permanency Details screen appears.
Enter a detailed explanation in the first narrative field; Document the steps taken to find an adoptive home, relative, legal guardian, or other permanent placement for the child.
If you agency has made specific recruitment efforts, describe them in the rest of the fields on this screen.
Click Save. You are returned to the Case Plan Topics screen.
Click Next Task below to continue.
