Participant Name

Click Person Search, then locate and select the person. Each participant must have a person profile in the system. This field is required.

Role

Select the role this participant fills, on the child's original or adoptive case. This field is required.

Agency/ Organization Affiliation

If an agency or other affiliation is entered on the person profile, the system displays that information. If not, you can enter the name of the agency or other organization this participant represents. You can enter up to 50 characters.
