Demographic information

Race

Select the check box next to the person's race. You can select more than one check box.

Hispanic/Latino

Select Yes or No to indicate whether the person identifies himself or herself as Hispanic/Latino.

Available Ethnicity

Select each ethnic origin that applies to this person, then click Add>>. The ethnic origin(s) you selected appears in the list box on the right.

Selected Ethnicity

This list box displays the ethnic origin(s) that applies to this person. To remove an ethnicity, select it, then click <<Remove.

Other Ethnicity

If the list boxes do not display the ethnicity that applies to this person, enter the person's ethnic origin.

Indian Tribe 1

Select the name of the primary tribe in which the person claims membership. These are ICWA values. This field is optional.

The system alerts the supervisor and worker when Indian tribe information is entered.

Indian Tribe 2

Select the name of a tribe if the person claims membership in another, secondary tribe. These are ICWA values. This field is optional.

Primary Language

Select the first, or primary,  language the person speaks.

Secondary Language

Select a second language the person speaks.

Interpreter Needed

Select this check box if the person needs an interpreter.

Religion

Select the person's religious affiliation.

Other Religion

If you selected Other in the previous field, enter a description of the person's religion.

Deceased

Select this check box if the person has died.

Deceased Date

If the person is deceased, enter the date on which he or she died. Use the format MM/DD/YYYY.
