MFE Date

Enter the effective date of the MFE. Use the format MM/DD/YYYY. This field is required.

Originating School District

Select the school district that initiated the MFE. This field is required.

Other

If you selected Other in the Originating School District field, enter the name of the school district. You can enter up to 50 characters. This field is required when you select Other.
