Domestic address details screen

This screen allows you to enter a U.S. address. Address information you entered as search criteria appears on this screen automatically.

During the search, the system attempts to validate the address. Addresses that have been saved and validated cannot be deleted. The system validates all addresses in SACWIS to ensure that they are real addresses and comply with the U.S. postal format. If an address is entered in error, contact the help desk for assistance.

Once the address has been validated, you can update only non-key fields, including the fields for environmental hazards, law enforcement jurisdiction, neighborhood name, and directions. You can also update the school district name, county, geo code, or census tract but only if the system could not validate this information.

Fields on this screen

Address	Enter the corrected street address. You can enter up to 200 characters in each address field.
Primary Address	Select this check box if this is the person's primary address. A person profile may have only one primary address. Clear the check mark if this is not the person's primary address.
City	Enter the name of the city. You can enter up to 50 characters.
State	Select the state.
Zip	Enter the zip code in the first field and the zip+four code, if any, in the second field.
Address Type	Select the type of address. This field is required.
C/O	If correspondence is sent to this person in care of another individual, enter that individual's first and last name. You can enter up to 250 characters.
Effective Date	Enter the date this address was in effect. This field is required.
End Date	Enter the date this address was no longer in effect.
Delivery Detail	Enter any additional information or delivery instructions. You can enter up to 250 characters.
