Elementary School	If the child will attend an elementary school, enter the name of the school. You can enter up to 250 characters. You must complete school information before submitting this home study for approval.

Middle/Jr. High School	If the child will attend a middle or junior high school, enter the name of the school. You enter up to 250 characters. You must complete school information before submitting this home study for approval.
